In order to complete the registration process, parents must complete and have the following forms on file BEFORE your child will be allowed to attend:


1. State of Michigan Child Information Card

2. State of Michigan Child In Care Statement / Receipt

3. State of Michigan Medication Permission Form

4. Completed Childcare Contract

5. Signed General Consents Form

6. Signed Notifications and Advisories Form

7. Completed Application for Food Program

8. A copy of the child’s current medical insurance card

9. Completed “All About Me” child information form

10. A copy of the parent/guardian's photo ID

11. Documentation that immunizations:

· Have been completed

· Are in progress

· Are not being administered due to religious, medical, or other reasons based on the parent's signed waiver


We do not charge any registration fees or deposits. In order to secure your child’s enrollment space you are required to pay the first week of tuition and submit all of your completed documents the Friday before care is scheduled to begin. Any available enrollment spaces will be filled, we will only hold an enrollment space if we have a completed enrollment packet and the first week’s tuition payment. If you are securing an enrollment space for a future date and you later decide not to enroll your child. The payment for the first week of tuition WILL NOT be refunded.
